We encourage our residents to embrace their end-of-life journey, making final wishes come true and healing each day to be full, meaningful, pain and anxiety free. Care will be personalized to fit that of each individual and their family.

In partnership with a hospice company of your choosing, we will develop a person centered plan of care that will reflect your medical, spiritual, emotional and physical needs in a respectful manner.

All of our suites are private and The Grace Space provides a fully electric hospital bed and a 55 inch television and TV services as well as a wardrobe/closet space for your personal belongings. We encourage patients to make their suite their own by brining in their favorite items from home (recliners, bedding, photos and décor for the walls), anything that makes your suite your home. Each suite has it’s own private bathroom and we have two beautiful spa spaces for you to shower or bathe in a relaxing whirlpool tub.

The Grace Space provides a beautiful and comfortable space for family and friends to participate in their loved one’s care. We offer beautiful indoor and outdoor spaces for families to gather and spend previous memory making moments together.

Our beautiful outdoor spaces provide opportunities for animal therapy, gardening, safe outdoor spaces to spend time with loved ones, meditate or just enjoy the fresh air and sunshine. Our landscape makes for the perfect setting to enjoy life to it’s fullest. Whether it’s sledding down the hill in the winter or planting a beautiful garden of vegetables and flowers in the spring, our desire is to fulfill the physical, emotional and spiritual needs of every individual as they embrace their journey.

The Grace Space provides onsite lodging for our families to allow them to stay close to their loved ones and participate in their care. We encourage families to stay with patients in their suites, cook meals, share in the holidays and special occasions and be actively involved in their daily care as desired. The Grace Space has two onsite bed and breakfast suites in our lower level that can be utilized by families coming from out of town which includes; a kitchenette (community), on-suite bathroom, living room, prayer and play room and space to breathe.

The Grace Space care team will assist you in living every day that we are given to it’s fullest, directed by each individual we serve. Partnering with the hospice/palliative agency of your choice, together we will educate our patients and families and openly discuss the end-of-life process, so as to be transparent, work through fears, restore and repair relationships, fulfill unmet needs, and assist in a peaceful, active and life filled journey.

Alongside our patients and their families, we will establish and end of life care model based on the idea that each individual is a fully realized person, with ideas, thoughts, experiences and needs that matter. Care will be driven by each individual and their family. Whether it be a religious preference, cultural belief or practice, or family tradition, it will be recognized by our loving staff and woven into the care of those we serve.
